Grounding and Retrieval Faithfulness
Grounding: tying claims to evidence
A language model's parametric knowledge — whatever it absorbed during pretraining — is fixed, often stale, and prone to confident-sounding fabrication. Grounding means constraining a model's output to be traceable back to specific, verifiable source material, typically retrieved documents, rather than relying purely on what the model 'remembers.' In a RAG (Retrieval-Augmented Generation) system, grounding is the entire point: retrieve relevant passages, hand them to the model as context, and generate an answer that should be derivable from those passages alone.
Faithfulness vs relevance: two different failure modes
RAG quality has two largely independent axes. Relevance asks whether the retrieved documents actually contain information useful to the query — a retrieval failure. Faithfulness (sometimes called groundedness) asks whether the generated answer is actually supported by the retrieved context, given that the right documents were retrieved — a generation failure. A model can be handed perfectly relevant documents and still hallucinate a claim not present in them, because nothing architecturally forces the decoder to only use the provided context; it can always fall back on its parametric priors.
Retrieval success doesn't guarantee faithful output
Measuring faithfulness: NLI and claim decomposition
A common evaluation pattern decomposes a generated answer into atomic claims, then checks each claim against the retrieved context using a Natural Language Inference (NLI) model or an LLM-as-judge prompt: does the context entail, contradict, or say nothing about this claim? Faithfulness score is typically the fraction of claims that are entailed. Frameworks like RAGAS formalize this into reusable metrics (faithfulness, answer relevance, context precision, context recall) so pipelines can be benchmarked and regression-tested as prompts or retrieval settings change.
def decompose_claims(answer_text, llm_call):
"""Ask an LLM to split an answer into atomic, checkable claims."""
prompt = f"List each distinct factual claim in this answer as a separate bullet:\n{answer_text}"
return llm_call(prompt) # returns a list of claim strings
def check_entailment(claim, context, nli_model):
"""Returns 'entailed', 'contradicted', or 'neutral'."""
return nli_model.predict(premise=context, hypothesis=claim)
def faithfulness_score(answer_text, context, llm_call, nli_model):
claims = decompose_claims(answer_text, llm_call)
if not claims:
return 1.0
entailed = sum(1 for c in claims if check_entailment(c, context, nli_model) == "entailed")
return entailed / len(claims)
# A faithfulness_score below ~0.8 on a representative eval set usually signals
# the generation prompt needs stronger "only use the provided context" constraints.Engineering for faithfulness
Several levers improve faithfulness in practice: explicit prompting that instructs the model to answer only from the provided context and say 'not found in context' otherwise; citation-forcing output formats that require inline references to specific source chunks (which makes unsupported claims visually obvious and easier to catch); reranking retrieved chunks so the most relevant evidence sits closest to the question in the prompt (models attend less reliably to information buried in the middle of long contexts); and post-hoc verification passes where a second model call checks the first's output against the sources before it's returned to the user.
Failure mode diagnosis
| Symptom | Likely cause | Fix |
|---|---|---|
| Answer is wrong and no relevant doc was retrieved | Retrieval failure | Improve embeddings, chunking, or reranking |
| Right doc retrieved, answer still wrong/unsupported | Faithfulness failure | Stronger grounding prompt, citation requirements |
| Right doc retrieved but ignored | Context placement/attention issue | Reorder context, shorten prompt, rerank |
What's Next?
Grounding a single-turn answer in retrieved text is the foundation, but real applications increasingly chain multiple retrieval and generation steps together into agentic workflows, where faithfulness has to be maintained across an entire sequence of decisions rather than one response.
